Release: (a) releases Blue Waves Swim Club from all claims and liabilities whatsoever arising from participation in or attendance at Blue Waves Swim Club current or future programs by the undersigned, the undersigned child(ren) or any associated spectator(s);

 

 

Refunds/Credits: (b) A refund will be provided if: Notice of cancellation is received two weeks prior to the starting day of the session, provided we are able to fill your spot. No other refunds will be made, as your spot in the class has been reserved throughout the session(s). A credit (less than $125 administration fee) will be given if notice of cancellation is received during the session, and is for medical reasons and is supported by a medical statement. The amount of the monetary credit, as applicable, for each student will be equal to the paid tuition fee less a pro-rated portion of the lessons taken, used or elapsed prior to receipt of written notice of withdrawal, as applicable, including any lessons scheduled to occur less than 24 hours from receipt of notice of withdrawal and a per student administration fee of $125.

 

Make-up Policy: (c) There is no refund for swimmers that miss practices for any reason. Make up options may be provided upon space availability.

 

Parents’ responsibilities: (d) Children must be supervised by their parents in change rooms at all times. If a parent is unable to supervise his/her child then the parent agrees that Blue Waves Swim Club is not responsible for his/her child. In consideration of the Blue Waves Swim Club permitting me or the person listed above to participate in fitness activities. Parents agree to release and discharge and to indemnify and save harmless the Club from and against all claims or proceedings by whomsoever made or brought, in respect of any costs, losses, damage or injury arising by reason of my/their participation in such activities, or by reason of the provision of medical care to me/them. Parents further authorize Blue Waves Swim Club to obtain such medical care to the person listed above, as it may deem necessary in the event of injury or otherwise, and agree to pay for all expenses incurred thereby.

 

Information and Registration: (e) The information in the registration Form is true and correct; and the undersigned acknowledges that he/she has read and understood all policies and information contained in this Authorization, Release and Acknowledgment form and registration form.
